Massage New Client Consent form

Non-refundable $25 deposit

Use this to pay a deposit if unable to hold the massage in the massage appointment booking app after filling out consent form. It is our policy to charge a deposit of $25 before your appointment starts if service is up to 90 minutes, otherwise the non-refundable deposit is $40 at pay link below, with the remaining balance to be collected before service once the mobile massage therapy provider arrives or after if you are an on going past client. This non-refundable deposit of $25 is to assure that in the case the client is not present for the therapeutic massage therapy appointment, or is late, or there is no parking, or the drive takes longer than 30 minutes by another 30 minutes, that the massage therapist service provider is compensated.

Use this link if unable to book on the massage booking app, in order to hold your appointment time. Text/call for available times after filling out consent form. For all services greater than 90 minutes a non-refundable deposit of $40 is required. The remaining balance of service is collected at time of service at client's service address by the therapeutic massage therapy provider. This non-refundable deposit also applies to any new client who enters the incorrect address on their service address even if only by one digit or zip code or misspelled street name. We have to insure all mobile massage therapy providers are safe, and nobody has access to any client's personal information.

Event & Special Occasion Group Massage 3-5 hours

If you are planning an amazing event to show your staff that you care about their health, or your family at the reunion or special occasion that you want them to be relaxed and feeling free, why not get a group massage? With a timed limit of 3 or 5 hours you can have a massage for each guest who signs in person the consent form that you initialize and print to bring to event for up to 50 people, or print more pages. Massage can be like a tool much like a massage hyperpercussion machine to help relax each guest by breaking apart their adhesion lesion knots and tight backs from postural changes. This consent form will need to be filled in for each event by the event planner and paid up front. Look for the special occasion massage for guests in our appointment scheduler below.

Updated Receipt following massage and body work session

Now you can reimburse yourself for wellness, maintenance, post-surgery, or active care treatment plans from your massage and body work appointments to help manage your care and save you money. Not all massage and body work is reimbursed by all insurance, but most are when in active care, using allowed CAM visits, or under a doctor's prescription or advised note for helping pain and healing after surgery or to manage specific health condition.

Refund Policy on therapeutic massage therapy, mobile massage therapy, etc

There is a small deposit required of all massages that is non-refundable if not cancelled before the start of the day or as transportation fee if no parking or client is not at massage address at time of appointment. The one hour massage session requires a $25 deposit, the 90 minute massage and all other require a $40 deposit. The remaining balance for the massage appointment is to be collected at the time of service and includes all prices in the listed prices and descriptions in the massage order form. Tips greatly appreciated but must be cash after service completes or Zelle. Each appointment is booked with a 30 minute gap after each massage, so if a couples massage, it is in tandem unless two massage therapists can work at the same time in one location. But you have to order the couples massage or one massage then wait 30 minutes and the other massage or contact your massage negotiotor to manually book the massage for you on the contact page of this site.

Refunds are refunded to the credit card used to book the appointment within 2-7 business days.

Group event massages at 3 or 5 hours are refunded fully if cancelled within 2 days, but are only scheduled with payment in full. If the same day cancellation, then 60% is refunded due to the amount of time it takes from availability for 4-6 hours that massage could be scheduled.

There is a refund on bundled packages like our monthly membership, the manual lymphatic drainage/lipocavitation package, and bundled family massage packages when the complete package was unable to be provided for unexpected circumstances of the client or clients.

All bundled packages that offer a discounted rate can be refunded for massages not used after deducting the regular priced savings to each used massage from the balance remaining.
All bundled packages have a time limit. The family massage package is only for the day within 4 hours as it is booked as a four hour service. The manual lymphatic drainage/lipocavitation package expires 90 days after initial purchase. The monthly memberships expire 90 days after the month purchased.

There is not a refund on the 20 minute spot treatments at $45 because there is a policy to test out the first 20 minute of massage on each massage and be refunded the amount past the non-refundable deposit. So, if you think you might not like the well trained massage therapist's skill or pressure, you should buy a full hour or 90 minute service and pay the non-refundable fee of $40 for massages 90 minutes or more, or $25 for less than 90 minutes as the 1 hour massage. You will only receive the first 20 minutes and the rest of the massage cancelled and remaining paid balance refunded in 5-7 days or however long it takes both banks to communicate as all transactions are via credit card or zelle.

A massage appointment will be cancelled and the non-refundable deposit forfeit toward a massage for any items in consent form being violated or disrespect of masseuse as a 2nd class lower citizen unworthy of hospitable treatment and safety in delivering massage services that heal.
